The purpose of the 4C: Chronic Condition Care Collaborative, hosted by the South Carolina Office of Rural Health (SCORH), the South Carolina Department of Health and Environmental Control (DHEC) and the American Society of Hypertension (ASH), is to provide health systems with the knowledge and skills to improve diabetes and cardiovascular disease care in clinical settings. This is a data driven quality improvement (QI) initiative that uses evidence-based interventions to optimize operational efficiency and promote patient self-management.
