South Carolina’s SC Patient Centered Primary Care Alliance is a collaborative effort of:

  • The South Carolina Department of Health and Human Services
  • The South Carolina Department of Health and Environmental Control
  • The South Carolina Medical Association
  • The South Carolina Primary Health Care Association
  • The South Carolina Office of Rural Health

The Alliance aims to improve the health and patient experience by implementing the Patient-Centered Medical Home (PCMH) model statewide. This team-based approach led by a primary care physician, nurse practitioner or physician assistant reinforces and rewards coordination between primary care and other practitioners to provide and monitor patient care. It engages patients in their own care and treatment and is supported financially by insurance companies.

Services Provided

South Carolina’s SC Patient Centered Primary Care Alliance hosts two workshops annually focused on assisting South Carolina’s PCMH and non-PCMH recognized practices in developing quality improvement skills and improving the delivery of care for patients.
