Practice Transformation Consultant

The Practice Transformation Consultant will be responsible for working with ambulatory practices, hospitals, specialty groups and other community and professional service organizations to meet specific grant/contract deliverables related to quality improvement, performance improvement, practice transformation, and quality measurement/reporting . The Consultant will work with healthcare organizations on chronic disease improvement, medical home development, PCMH Recognition, public health program implementation, systems optimization and quality improvement to help practices achieve improved patient outcomes, office efficiency and satisfaction for the patient and the health care team.

Program Manager, Community Health Improvement 

Lexington, SC SCORH office
The Program Manager on our Community Health Improvement team will provide technical assistance for current activities related to population health. The Manager will work with partner community coalitions and organizations to manage and coordinate programs and services.

Infant Early Childhood Mental Health Consultant

Orangeburg, SC SCORH office
The IECMH Consultant will be part of a team developing an IECMH Consultation Network in South Carolina. The IECMH Consultation Network will support young children, their families, and the professional “helpers” in their lives (child care teachers, social workers, etc.). The consultant will use his/her knowledge of early childhood development coupled with an understanding of family dynamics and relationship building at an individual/family, provider and community level, to support the social-emotional growth of very young children.
